  • As an Adult-Gerontology Primary Care Nurse Practitioner (APRN, AGPCNP-BC), I am licensed and board-certified to serve as a primary care provider. I can:

    ·        Diagnose and treat medical conditions

    ·        Prescribe medications (including sleep medications when appropriate)

    ·        Order and interpret labs, imaging, and other diagnostic studies

    ·        Provide preventive care, chronic disease management, and health counseling

    ·        Write orders for therapy, home health, and specialty referrals

    The main difference between a nurse practitioner and a physician is our training model: physicians are trained in the medical model, while nurse practitioners are trained in the nursing model, which emphasizes patient-centered care, education, and wellness alongside medical treatment. Both NPs and physicians can serve as your primary care provider, but NPs bring a unique emphasis on holistic, relationship-based care.

  • Memberships: $4,500/year (or $375/month). Monthly patients must keep a card on file for automatic billing.

    Medicare patients: I will bill Medicare (and secondary insurance if you have it) for medically necessary visits and procedures.

    ·Non-Medicare patients: Visits are billed directly to you at the rates above; I can provide a superbill for potential reimbursement.

  • If you have private insurance other than Medicare, I am considered out-of-network. I can provide a superbill that you may submit to your insurer for potential reimbursement, depending on your plan.

    • One annual comprehensive physical exam

    • Annual laboratory panel (covered at Jupiter Medical Center)

    • Priority and same-week appointments

    • Direct communication (phone, text, email)

    • Transitional care coordination after hospital stays

    • Exclusive discounts on wellness services such as IV therapy, injections, and weight management programs

  • Medically necessary visits will be billed to Medicare/insurance. Cash-based services such as IV infusions, vitamin injections, and weight management therapies are offered at member-discounted rates. House calls for convenience or non-medical purposes are also billed separately.

  • Covered: If a home visit is medically necessary (for example, a patient is homebound or unable to travel), it may be billed to Medicare.

    Not covered: If the home visit is elective or for convenience (for example, you prefer not to come into the office), it is a cash service billed separately.

    Cash-pay convenience visits are subject to a separate travel/driving fee determined by location

  • Medicare is always primary. Your secondary plan may cover co-pays, deductibles, or coinsurance left over after Medicare.

    Membership fees are not covered by Medicare or secondary insurance — those are always out-of-pocket.

  • On the rare occasion I am away, I will notify my patients in advance and arrange for backup coverage from a trusted local provider (Dr. Gocke) or ensure you have appropriate access to urgent care or specialists as needed.

  • No — my patients have direct access to me. Calls, texts, or secure messages come straight to me, not through a call center.

  • No — I do not provide inpatient hospital rounding. Hospital care is managed by hospitalists at Jupiter Medical Center.
    ✨ However, I play an active role in transitional care:

    • I track your hospital status

    • Coordinate with the hospital team

    • Review your discharge instructions and medications

    • Ensure you have timely follow-up with me after your hospital stay

    • This continuity helps reduce readmissions and ensures a smooth recovery.

  • Standard office hours: Monday – Thursday, 9am–5pm.
    Fridays are reserved for emergent patients, transitional care follow-ups, and practice management.

  • Item descriptionFlorida law allows me to prescribe controlled substances within certain limits. To keep patients safe and compliant with state law:
    - Pain medications (opioids, Schedule II): up to a 7-day supply only. Long-term pain management must be handled by a pain management specialist.
    - Benzodiazepines (Xanax, Valium, Ativan): short-term use may be prescribed when appropriate, but chronic use must be managed by psychiatry.
    - Stimulants (Adderall, Vyvanse, etc.): may be prescribed short-term, but long-term use requires psychiatry oversight.
    - Sleeping medications (Ambien, Lunesta, etc.): can be prescribed for routine use when clinically appropriate.
    - All other routine medications (blood pressure, diabetes, cholesterol, thyroid, antibiotics, etc.) are fully within my scope.

  • Memberships are for 12 months. If paying monthly, you may stop future payments with at least 30 days’ written notice, but fees already paid are non-refundable. Annual upfront memberships are final and non-refundable.


    When terminating, you must provide the name of a designated primary care provider (PCP) so records can be transferred. After termination, I will no longer be responsible for ongoing care or prescription refills.

  • Dr. Gocke and I are separate practices. I operate independently as Deeper Well Concierge, but I collaborate with him as a mentor and consultant. Your membership with me does not overlap with his.

  • Routine labs for the annual physical are included and performed at Jupiter Medical Center. Additional labs or imaging ordered as part of medical care will be billed to Medicare/insurance.

  • Members receive priority same-week visits. Same-day house calls for urgent but non-emergency issues may be available for an additional fee. For true emergencies, please call 911.

  • I believe in quality over quantity. By keeping my membership panel intentionally small, I can spend more time with each patient, provide continuity of care, and develop personalized wellness and treatment plans.

  • We are in the process of hiring a medical assistant (MA) who will assist with scheduling, clerical support, and patient flow to ensure smooth operations.

    Additionally, our new EMR system will include online scheduling, secure messaging, and a patient portal for labs and visit summaries.

  • Yes! I offer telemedicine visits for appropriate concerns so you can access care from home or while traveling.

  • Office visits: $200–$250 (depending on complexity)

    House calls (non-medical/convenience): $300+
